SPORTS HEALTH HISTORY UPDATE
A health history is needed if the Pre-Participation exam date was greater than 90 days from the start of the practice. For example, the student's original exam was June 1 and the first practice is December 1. If there has been an injury since the date of the original exam, then medical clearance is needed.
FTS Sports Permission to participate - completed by parent/guardian; must be submitted for every sport an athlete plays.
NJ Health History Questionnaire - completed by parent/guardian every season unless the physcial was dated less than 90 days ago.

Sports Related Concussion and Head Injury Fact Sheet - Submit once a year; covers all sports seasons.
