SPORTS HEALTH HISTORY UPDATE
A health history is needed if the Pre-Participation exam date was greater than 90 days from the start of the practice. For example, the student's original exam was June 1 and the first practice is December 1. If there has been an injury since the date of the original exam, then medical clearance is needed.

• FTS Letter to Student Athletes & Parents_Guardians (Grades 5-8) as of 2024-2025
• Preparticipation Physical Evaluation Medical Eligibility Form
• FTS Sports Permission to participate  - completed by parent/guardian; must be submitted for every sport an athlete plays.
   
• NJ Health History Questionnaire  - completed by parent/guardian every season unless the physcial was dated less than 90 days ago.
   
• Opioid Drugs Fact Sheet Sign Off  - Please read the  Opioid FactSheet  before signing. Submit once a year; covers all sports seasons.
   
• PPE Athletic Physical Form
   
• Sports Related Concussion and Head Injury Fact Sheet  - Submit once a year; covers all sports seasons.
   
• SportsRelatedEyeInjury  (There is no sign-off form.)
   
• Sudden Cardiac Arrest Pamphlet Sign Off  - Please read the  Suddent Cardiac Death in Young Athletes Pamphlet  before signing. Submit once a year; covers all sports seasons.
