BEGINNING OF THE SCHOOL YEAR

At the beginning of the school year, parents/guardian must fill out an  Emergency Form , Annual Student Health Information Form , and Over-the-Counter Medication Form . This is done in the Genesis Parent Portal .

SPORTS HEALTH HISTORY UPDATE

A health history is needed if the Pre-Participation exam date was greater than 90 days from the start of the practice. For example, the student's original exam was June 1 and the first practice is December 1. You can submit a Short Health History form . If everything is negative, I can sign off. If there has been an injury since the date of the original exam, then medical clearance is needed.
